The records included Start of Care Oasis forms along with 485 Recertification Oasis forms, Case Conferences, 60-Day Summaries, Discharge Summaries Oasis forms, Skilled Nursing Visits, and Aide Care Plans.

The RN’s conduct resulted in incomplete medical records that were likely to deceive subsequent caregivers who relied on the information while providing care to the patients.

Because of this, the RN was summoned by the Texas Board of Nursing. During the hearing, the RN admits that she did not completely document various patient records while working for the medical facility during the months of July 2014 and August 2014.

The RN was given the chance to defend her license but she failed due to lack of proper defense from a nurse attorney. Unfortunately, her license was disciplined.
