On or about February 26, 2016, while employed as a Registered Nurse at a hospital facility in Mesquite, Texas, the RN engaged in the intemperate use of Amphetamines, Methamphetamines, and Cocaine, in which he produced a specimen for a drug screen which resulted positive for Amphetamines, Methamphetamines, and Cocaine. In response, the RN submitted sixteen (16) negative drug screens for the time period of March 12, 2016, through March 16, 2017.
