The RN also failed to appropriately identify the unlicensed personnel responsible for the aforementioned delegated tasks, training received, verification of competency, and monitoring requirements. The RN’s conduct created an inaccurate medical record and was likely to injure the patient in that subsequent caregivers would not have accurate and complete information on which to base their care decisions.
In response to the incident, the RN states all staff is in-serviced a minimum of once a year on documentation of intake, output, and vital sign measurements when said staff are delegated for medication administration. The RN further states staff were specifically trained in the patient’s specific dietary needs.
